Associate (Supplier) Membership Application

Annual Dues $1,650.00

I hereby apply for membership as an Associate Member. To qualify in accordance with Article II, B.2.(b) of the Bylaws an applicant must:

Have been engaged in the gasket fabricating industry a minimum of one (1) year.
Have an annual volume of at least one million dollars ($1,000,000 U.S.) in sales of raw materials, equipment, services or supplies of any form or description to gasket fabricators.
