Gasket Fabricators Association - Fabricator Membership Application

APPLICATION FOR FABRICATORS MEMBERSHIP

(Annual Dues $950.00)

I hereby apply for membership as a Fabricator Member. I qualify in accordance with Article II,B.1(b)of the Bylaws as follows:

An application must:

1. Have been engaged in the gasket fabricating industry a minimum of one year.

2. Do an annual volume of at least one million dollars ($1,000,000) in the sale of fabricated gaskets, including metallic gaskets and gaskets which are perforated, encased or enveloped.

For the purposes of qualifying for membership, die-cut gaskets shall include metallic gaskets and gaskets which are perforated, encased or enveloped.

Any business entity which is a supplier of material to gasket fabricators is not eligible for Fabricator Membership.

Verification of the above criteria will be aided by inclusion of three (3) major raw material suppliers for reference by the GFA Office.

Please remit a print-out of this frame and return with the following:

A.  A check for one year's dues made out to GFA.  Your dues will be prorated according to
your month of application if received after April.  A credit will then be issued against your
dues for the following year.

B.  A brief profile of your company.
